Why post-thrombotic syndrome develops
When a deep vein clot forms, it can scar the vein wall and damage the one-way valves that keep blood moving back toward the heart. Even after the clot itself resolves, that damage can leave blood pooling in the leg — a form of chronic venous insufficiency. This raises pressure in the veins and produces the ongoing symptoms of PTS. Several factors can make PTS more likely:
- A large or extensive DVT, especially in the thigh or pelvis
- More than one clot in the same leg
- A clot that was not fully treated or that took a long time to improve
- Being overweight, which adds pressure on the leg veins
- Existing venous reflux or valve weakness before the clot
Symptoms of post-thrombotic syndrome
Symptoms usually affect the leg that had the DVT and often build through the day or after long periods of standing. Common signs include:
- Aching, cramping, or a heavy, tired feeling in the leg
- Swelling in the calf or ankle that worsens as the day goes on
- Itching, tingling, or a sense of tightness
- Darkened or discolored skin around the ankle
- Varicose or bulging veins in the affected leg
- In more advanced cases, skin breakdown or a venous ulcer near the ankle
