What is post-thrombotic syndrome?

Post-thrombotic syndrome (PTS) is a long-term complication that can follow a deep vein thrombosis (DVT), when a past clot damages the vein and its valves and leaves the leg with lasting swelling, aching, and heaviness. At Charming Skin & Vein Clinics, we evaluate the affected veins and build a plan to ease your symptoms.

Why post-thrombotic syndrome develops

When a deep vein clot forms, it can scar the vein wall and damage the one-way valves that keep blood moving back toward the heart. Even after the clot itself resolves, that damage can leave blood pooling in the leg — a form of chronic venous insufficiency. This raises pressure in the veins and produces the ongoing symptoms of PTS. Several factors can make PTS more likely:

  • A large or extensive DVT, especially in the thigh or pelvis
  • More than one clot in the same leg
  • A clot that was not fully treated or that took a long time to improve
  • Being overweight, which adds pressure on the leg veins
  • Existing venous reflux or valve weakness before the clot

Symptoms of post-thrombotic syndrome

Symptoms usually affect the leg that had the DVT and often build through the day or after long periods of standing. Common signs include:

  • Aching, cramping, or a heavy, tired feeling in the leg
  • Swelling in the calf or ankle that worsens as the day goes on
  • Itching, tingling, or a sense of tightness
  • Darkened or discolored skin around the ankle
  • Varicose or bulging veins in the affected leg
  • In more advanced cases, skin breakdown or a venous ulcer near the ankle

If you have had a DVT and notice these changes, an evaluation can show whether venous reflux is contributing and what can help.

How we evaluate and treat it

Because PTS follows damage beneath the skin, we start by understanding what is happening in the deep and surface veins before discussing treatment. Dr. Abboud brings more than 20 years of experience caring for veins, and care begins with a clear diagnosis and a plan matched to your symptoms.

Lifestyle steps — staying active, elevating the leg, and managing weight — also help lower pressure in the veins and support long-term comfort.

Common questions

When should I see a vein doctor after a DVT?

It is worth scheduling an evaluation if, after a deep vein thrombosis, you notice lingering swelling, aching or heaviness in the leg, skin changes near the ankle, or bulging veins. These can be signs of post-thrombotic syndrome, and an ultrasound can show whether the valves and veins are still supporting healthy blood flow.

Is treatment covered by insurance?

Most PPO plans and Medicare cover treatment for symptomatic veins. Purely cosmetic vein work is self-pay. We are glad to review your symptoms and help you understand your coverage before any treatment.

What can I expect at a free vein screening?

Your screening is a relaxed, informational visit. We listen to your symptoms, look at your legs, and talk through whether an ultrasound and further evaluation make sense. There is no pressure — the goal is to help you understand your veins and your options.

Quick answer

Post-thrombotic syndrome (PTS) is a long-term complication that can develop after a deep vein thrombosis (DVT) damages the vein valves, leading to ongoing leg swelling, aching, heaviness, and skin changes. Care focuses on managing chronic venous insufficiency through compression therapy, an ultrasound evaluation of the affected veins, and a treatment plan tailored to your symptoms.

Medically informed care

Vein care at Charming Skin & Vein Clinics is led by Dr. Abboud, who brings more than 20 years of medical experience — a graduate of the University of Damascus with residency training at Cook County Hospital and John H. Stroger Jr. Hospital in Chicago. This page is for general education, reviewed for accuracy, and is not a substitute for an in-person evaluation.

Because PTS often follows a clot, we begin with an ultrasound vein diagnosis to map blood flow, and we can review related concerns such as deep vein thrombosis when planning how we help. Most PPO plans and Medicare cover treatment for symptomatic veins.
